I will support the Coach in ensuring the guidance is followed.
I give my full consent for my child to take part in all football and fitness activities at Lil kickers Academy.
By signing I give my permission for any pictures/videos taken of my child to be used on any Lil Kickers Academy Social Media pages.
I understand that John Allitt shall not be liable for any damages arising from personal injuries sustained by any child while and during and/or from any Lil Kickers Academy sessions.
BY CONSENTING TO ALL OF THE ABOVE ON THIS FORM I AGREE TO ABIDE BY THE GUIDANCE AND RULES SET BY LIL KICKERS ACADEMY.
